Some provinces are controlled loosely by rebel glitch factions, but the majority of provinces are ruled with an iron fist by the evil pharmaceutical conglomerate VAXXCORP, with a vast army of hired mercenaries who manage the VAXX TOLLS that allow freedom of movement for the VAXXED—for a price.
